The builder is contractually accountable for build quality and specification. Often they don't achieve it. What nobody is doing is independently verifying the work for the owner, while it's still fixable.

Most major defects originate from construction execution rather than design intent. That means they are visible during construction, and preventable with rigorous inspection, provided someone is on site checking the work against the specification before it is covered up.
